PRE-REGISTRATION IS REQUIRED FOR ALL OF OUR
CLASSES & CAMPS. NO WALK-INS, PLEASE.
Our Sunday Class...runs year-round for boys/girls, grades 3 - 12
Location: Chicago H.S. for Agricultural Sciences, 3857 W. 111th St., Chicago, 60655
Program Details:
Grades 3 - 5: 9:00am - 10:30am, $15/session
Grades 6 - 12: 9:00am -11:00am, $20/session
Payment Options:
Payment can be made monthly or week by week. Checks are made payable to: ELITE
VOLLEYBALL PROGRAM, INC.
If you pre-pay and need to cancel, you must cancel prior to clinic to receive a credit
or refund.

Option 1: Fill out & return registration form prior to the start of the month
Option 2: Notify us weekly by the FRIDAY before the class via e-mail.
IHSA RULING...during the "school year", only 2 students/H.S. can attend a class
